Open Enrollment is Here! Here’s What You Need to Know:
Open Enrollment is your once-a-year opportunity to review your benefit options, enroll, and add/drop eligible dependents from coverage. It’s a great time to review your current coverage to ensure your benefits meet the needs of you and your family, as well as review and update your beneficiary information.
This is a passive enrollment for your 2025 benefits. You do not need to re-enroll unless you are electing and contributing to a Healthcare Flexible Spending Account (FSA), a Dependent Care FSA, or a Health Savings Account (HSA). All these benefit options require you to take action and re-enroll for 2025.

What’s Changing in 2025?
We’re excited to introduce a new medical plan option through Surest, a UHC company. The Surest Plan will be offered along with the PPO $1,500 and HDHP $3,300. Read about this new option below.
Surest Plan Highlights
- No deductibles or coinsurance: With fixed copays for all services, the Surest Plan eliminates deductibles and coinsurance.
- Transparent costs: The Surest Plan lets you see what you’ll pay before your doctor’s visit.
- Same UHC network: You’ll continue to have access to the UHC Choice Plus Network, giving you a wide range of healthcare providers. Note a different member services phone number and website.

IMPORTANT: If you are currently enrolled in the HDHP 6000 Plan OR you are a team member of SASS and do not make a new election, you will automatically be enrolled in the Surest Plan.
Note these additional updates for 2025
- HDHP $3,300: The deductible will increase to $3,300 (individual) and $6,600 (family), in line with updated IRS guidelines. The Plan will be called HDHP 3300.
- New ID cards: All employees will receive new medical ID cards – even if you did not change your coverage option.
- 2025 IRS HSA limits:
- Individual only: $4,300
- Family: $8,550
- Age 55+ or older: $1,000 catch-up contribution
401(k) match
- 25% match up to 6% of your contribution.
- We know the 401(k) is important to you and we are continuing to review our match formula to provide the best experience for our team members.

Enrollment Instructions
- Review your benefit elections in UKG to confirm your plan elections and information are accurate.
- Take a moment to look at the resources and share the information with your spouse. You can easily review benefits information on this website or check the Benefits Resource Center.
- Do you want to make changes to your current coverage? If not, no need to re-enroll unless you are re-electing a Healthcare or Dependent Care FSA. As good practice, we recommend you review and update your beneficiaries.
- Once you have completed your elections in UKG and clicked “Submit,” you can print/save your Confirmation page for your records.
